Stephen F. Austin Community Health Center (SFACHC) was established in 2008 to help provide healthcare services for the Alvin Community. The year 2010 was a big year for us, we initiated our Women’s Health Program, opened our Dental Clinic and also started our Behavioral Health Department. In 2011 SFACHC then began providing psychiatric services to residents in the county. With the expansion of services in Alvin, we soon began to see the need for services in other areas. In 2013 we launched our second site located in Freeport, Texas. The Freeport Community Health Center provides Adult Medicine, Women’s Health, OB/GYN, Mental Health and General Dentistry Services. After the opening of the Freeport Community Health Center, SFACHC then changed its name from Stephen F. Austin Community Health Center to Stephen F. Austin Community Health Network (SFACHN). We then renamed our Alvin location to Adoue Family Health Center. Local demand for services led us to open a 2nd site in Alvin to serve women and children. Our Women & Children’s Health Center opened in 2015, to provide Obstetrics/Gynecology and Pediatrics services. In addition to these services we provide integratged care for adults receiving care through Gulf Coast MHMR in Alvin, Angleton and Galveston, as well as onsite primary care services in 2 School Based Health Center's in Freeport and other collaborative partnerships. Dr. Yvette M. Poindexter M.D., FACOG OB/GYN
Yvette M. Poindexter, M.D. is a board certified Obstetrician-Gynecologist who is the Chief Medical Officer for Stephen F. Austin Community Health Network. She graduated from University of Texas Medical Branch in 1993 and Baylor College of Medicine in 1997. Since her arrival she has helped to implement two prenatal programs in the Brazoria County service area including Alvin and Freeport. She delivers Women's Health and Prenatal Care at both locations. A collaboration with Dr. Maximos and Dr. Parker enable our patients to deliver at Clearlake Regional and UTMB Angleton-Danbury Campus. Dr. Poindexter's greatest strength is her commitment to the mission of providing health care to all people without prejudice, in a caring manner, regardless of their ability to pay.
Dr. Shari Jackson M.D. Pediatrics
Dr. Shari Jackson is a general pediatrician who sees patients from newborn through young adulthood. A native of Texas, she grew up in San Antonio. She attended Trinity University for her undergraduate years and obtained her medical doctorate degree from the University of Texas Health Science Center at San Antonio in 1999. She completed her pediatric residency at the University of Texas Medical Branch in Galveston in 2002. Following residency she went on active duty and was an Army pediatrician at Ft Bragg, North Carolina where she was Officer-in-Charge of the Pediatric Clinic at Womack Army Medical Center and she obtained her board-certification in pediatrics. She has since practiced in both rural and suburban settings, in large and small practices in North Carolina and Connecticut. Prior to returning to Texas, she was a clinical teaching faculty member at the American University of Antigua School of Medicine in Antigua, West Indies. Her interests are preventive care and continuity of care. She strives to guide each child to be happy and healthy.
Dr. Antonia Way, MD
Dr. Antonia Way is a Family Physician who sees patients of all ages, from birth to geriatrics. She did her undergraduate and medical training at the University of New Mexico School of Medicine and went on to complete her Residency with the University of New Mexico Department of Family and Community Medicine. After completing her residency Dr. Way came to Houston and has been practicing here since. Her interests include preventative care, chronic disease management, and non-opioid management of chronic pain.
Dr. Prashant Gajwani, MD
Dr. Prashant Gajwani is a Psychiatry Specialist in Houston, TX. He graduated with honors in 1993 at Jawahar Lal Nebru Medical College Medical School. He did his residency in Psychiatry at the Cleveland Clinic Foundation. Having more than 24 years of diverse experiences in psychiatry and addiction medicine. Dr. Gajwani is affiliated with Methodist Sugar Land Hospital and cooperates with other doctors and specialists in many medical groups. He diagnoses and treats mental illness, such as depression, anxiety disorder, substance abuse and schizophrenia.
Dr. Gajwani is certified in Psychiatry from the American Board of Psychiatry and Neurology, with licenses from OH State Medical and TX State Medical.
Dr. Rick Ertell Behavioral Health
Rick Ertell earned his doctorate from Baylor University in Educational Leadership. He also earned an M.S. in Counseling and a B.S. in Political Science from the University of Houston-Clear Lake (UHCL). He is a Licensed Professional Counselor (LPC) and an LPC Supervisor. He is certified by the State of Texas as a Special Education Teacher, Counselor, Special Education Counselor, and Principal. He has served as an Elementary, Middle, and High School Counselor. In addition he has also served as a school administrator.
Currently Rick is the Director of Behavioral Health for Stephen F. Austin Health Network. Rick also serves patients as a therapist that provides screenings and assessments, group and individual counseling services and referrals for psychiatric care.
Rick is a loving husband to his wife Robin. Rick is active in the community having served on many boards and committees including Hospice Care Team and The United Way of Brazoria County. He is actively involved in the community and he has served as a volunteer at the Houston Area Women's Center.
Dr. Venkat Veerisetty, DMD Dentist
Dr. Venkat Veerisetty, DMD has served as the Dental Director of Stephen F. Austin Community Health Network since January of 2015. Prior to this Dr. Veerisetty served as an independent practitioner with offices in Pearland, Sugarland and Freeport. His endeavor in Freeport is what brought him to Stephen F. Austin Community Health Network. While in Freeport, Dr. Veerisetty provided services for individuals who lacked access to quality dental care. Dr. Veerisetty has established the Center’s relationship with the University of Texas Health Since Center – Dental School, where he served as an associate professor from 1995-2000. In addition to his degree as a DMD, Dr. Veerisetty also holds a PhD in Life Sciences and a Bachelors in Biology.
Dr. Clementine Ingabire, DDS Dentist
Dr. Clementine Ingabire joined Stephen F. Austin Community Health Network in 2014. She is a graduate of Howard University College of Dentistry in Washington where she graduated with a Doctor of Dental Surgery degree. Dr. Ingabire comes to SFACHN with a wealth of knowledge and experience that has propelled her to becoming a prefered dentist in the Alvin area. Dr. Ingabire is a native of Rwanda and is a proud wife and mother of two.
Dr. Lorena Chacon-Smith, DDS Dentist
Dr. Lorena Chacon-Smith is a bilingual dentist dedicated to providing quality care for our patients in a friendly environment. Dr. Chacon-Smith is a graduate from the University of Texas School of Dentistry at Houston. She served our country as a member of both the U.S. Army and U.S. AirForce Reserves after growing up in Freeport, Texas and graduating from Brazosport High School. In her free time Dr. Chacon-Smith enjoys fishing and camping with her husband Shaun, son William, and great dane Dana.

Lucia Tran PA Family Practice
Lucia Tran is a Graduate of Jefferson College of Health Sciences has been with Stephen F. Austin Community Health Network since 2012. Lucia Tran has been instrumental in the implementation of integrated mental health services throughout our organization. Her caring approach to health care has been the driving force to her success with our community.
Jessica Castaneda FNP Family Practice
Jessica Castaneda is a Graduate of University of Texas Health Science Center School of Nursing and has been with Stephen F. Austin Community Health Network since 2013. Jessica has overseen the implementation of many chronic disease projects and has been integral in the implementation of our Patient Centered Medical Home certification. Jessica is loved by her patients and her desire to meet the needs of her patients is what drives her forward.
Laura Gordon, FNP Family Practice/SBHC
Laura Gordon joined Stephen F. Austin Community Health Network in 2015. Laura has a degree in Human resource management from Stephen F. Austin State University, a Nursing degree from UT Arlington, and a Master's in Nursing from Walden University. Laura is a wonderful wife and mother of 3 lovely daughters, Dorothy 27, Anna 25 and Chloe 14.
Elizabeth Adeseko is a Licensed Clinical Social Worker and counselor and has been with Stephen F. Austin Community Health Center for 3 years. She has a Master's Degree in Social Work from Loyola University Chicago and is a native of Wisconsin. She enjoys working with children, adolescents, families and adults on issues such as depression, anxiety, bullying, and anger. She enjoys helping clients find their greatest potential and to know they are not alone.
